PKM Clinician – Social Worker

Overview

Required:

  • BSW or MSW or equivalent. LSW minimum license in Ohio.
  • One year experience working with individuals as a mental health clinician.
  • Maintain 30 CEU for licensure

As a social worker you are an important part of the services that PKM Life Transitions provides to individuals living in the community.

We provide mental health services to agencies and insurance company referrals. Behavioral health is the basic path that we will follow using a variety of treatment methods including but not limited to CBT, Talk Therapy, Trauma Therapy and others that you may be personally trained to use.

You must have one year experience working with others in some type of social program.

You must maintain your LSW or LISW, comply with the background (BCI, FBI) checks and report any issues concerning your license to the office immediately. If it is no longer valid or you are being suspended, your employment with PKM with terminate immediately.

Within our contracts with the AAA, Passport and MyCare Ohio insurance companies we do not provide case management services, nor do we provide resources for medical equipment, meals, transportation, day programs or any other services that include community resources. We should note the client's needs and contact the case manager to let them know of the need so they can then put things in place to assist the individual.

Professional services:

  • Establish a visit schedule with clients assigned
  • Develop a Care Plan and review with the client every 90 days updating as needed
  • Provide therapy to the client and/or family if they are having issues with coping, grief and other mental health issues
  • Contact the Case Manager with client needs and concerns immediately
  • Review information and send it to the office timely
  • Intake forms within 7 days of initial meeting
  • Care plans to initially be sent with the intake and then every 90 days after that upon review. Signatures and follow up are required related to changes to the care plan
  • Missed visits must be reported within 24 hours
  • Client concerns and injuries or other things that impact them must be reported to the case manager as soon as you learn about them
  • Establish boundaries to ensure excellent care
  • Provide updates and information to the medical staff as needed to benefit the client and provide continuity of care.
  • Establish and follow all HIPAA rules and regulations to ensure privacy

The NASW code of ethics will be the primary tool that will be used to measure the quality of services and individual staff's conduct.
